Questions, answered

Will I lose my game saves?

No. GameCleaner backs up local saves before any removal, and Steam Cloud saves are never touched. Uninstalling also keeps the game in your Steam library — reinstall anytime.

Is deleting shader cache safe?

Yes. Shader cache is derived data the game rebuilds on next launch. You may see brief stutter the first session after, then it's gone. Nothing irreplaceable is removed.

How is this different from Steam's built-in Storage Manager?

Steam shows size per game but not how long since you played, doesn't back up saves, doesn't clean caches, and is Steam-only. GameCleaner flags stale games, estimates reclaimable space, backs up first, and (soon) covers Epic, GOG and Xbox.
